When you check in to Gottlieb Memorial Hospital, please bring your insurance card, a photo ID and your doctor's orders. It is also very important that you bring information about an emergency contact person including their name, address and telephone number(s). If you don't want to wait on the day of your test or surgery, you can come in a few days ahead to preregister.
If you have a Living Will or a Durable Power of Attorney, you should share it with your family and bring a copy with you. These documents can help to ensure that your wishes regarding the use of certain treatments will be followed even if you are unable to communicate or make decisions due to illness, disease or accident. Also bring with you to the hospital: dentures, eyeglasses, a robe, non-skid slippers and a list of medications you are currently taking.
