Eligibility: Individual plan holders of certain UnitedHealthcare Medicare plans

Enrollment/Annual Fees: All fees are paid for by your insurance

Monthly Dues: Monthly dues are paid for by your insurance

Activation Code: Eligible members should contact their insurance company to retrieve their own unique activation codes which begin with the letter "A" or "S" and are followed by a nine digit number. Please bring your code with you to register for membership.

Enrollment: Our friendly membership services staff can help you become a SilverSneakers® member at no cost to you. Please be sure to bring your SilverSneakers® 16 digit membership ID number with you. The membership is provided as a free benefit to you through your insurance company.
