Report a Concern to GME

Please use the form below to report a concern. We take all concerns seriously and all submissions will be investigated by our GME office. If there are any immediate concerns, please contact Greg Ozark, M.D., Vice-President Graduate Medical Education, through our paging system. You can also contact the GME Office during regular business hours at 708-216-4533. Reports can be submitted anonymously, however be aware that anonymous reporting limits our ability to follow up with you individually for any additional information.
