It's very disrespectful of you to call a physician a provider. The American Medical Association and many other national medical body warn against such act. You cannot devalue 12-17 years for training to attain the title physician by calling one a provider. This also misleads patients who are unaware of who is treating them. Shame on you. You must change this survey.
