Letter of Good Standing Request
Letter of Good Standing Request Information:
- COMPLETELY fill out the Letter of Good Standing Request Form
- Ensure contact information for the recipient is ACCURATE
- Letters of Good Standing may not be given to the individual provider, they must go to a receiving EMS system or Employer
- Lack of recipient email address will result in a letter being sent via USPS (5-7 days)

Providers MUST be in good standing, in order to have a LOGS issued
(All monthly CE completed, Current AHA CPR Card on file, No ongoing disciplinary action)
