Letter of Good Standing Request


Letter of Good Standing Request Information:

  • COMPLETELY fill out the Letter of Good Standing Request Form
  • Ensure contact information for the recipient is ACCURATE
  • Letters of Good Standing may not be given to the individual provider, they must go to a receiving EMS system or Employer
  • Lack of recipient email address will result in a letter being sent via USPS (5-7 days)
  • Providers MUST be in good standing, in order to have a LOGS issued


    (All monthly CE completed, Current AHA CPR Card on file, No ongoing disciplinary action)
