Loyola Medicine Home Care provides in-home nursing and therapy care 24 hours a day, seven days a week for patients who are chronically ill or recovering from surgery or an illness. Partnering with the physician and family, the home care team helps the patient to reach health goals, prevent avoidable hospitalizations and transition from a hospital stay back to daily activities.
Loyola Medicine Hospice
Loyola Medicine Hospice cares for patients who have a serious, potentially terminal illness, providing support, comfort and compassion 24 hours a day, seven days a week. Patients are cared for in their own home, surrounded by the support of their loved ones. In collaboration with the patient’s physician, the hospice care team provides pain management and symptom control. Psychological, emotional, social and spiritual support is offered to patients and their families. The healing bereavement services program helps family members grieve the loss of a loved one.
