Loyola Medicine’s Diabetes Care Center provides comprehensive group and individual treatment programs for adults with diabetes.
The center utilizes a team approach to patient care, providing you with the expertise of many specialized health professionals. This includes board-certified endocrinologists, advanced practice nurses, certified diabetes educators and social workers.
The Diabetes Care Center at Loyola Medicine Hickory Hills is designed as a "one stop shop." Your coordinated visit will allow you to see all of the necessary physicians and services – such as diabetes education, endocrinology, ophthalmology and podiatry – all during the same visit.
You will discuss your results with your diabetes educator, who will work with you to schedule your upcoming appointments.
Loyola's diabetes program is recognized by the American Diabetes Association (ADA) and follows the ADA's standards of care. These standards are directed toward ideal control of diabetes and prevention of complications.
Services Offered
Loyola provides advanced, multidisciplinary care for patients with diabetes. In addition to physician evaluations and individual treatment programs, Loyola offers educational classes for diabetes patients.
These classes are taught by nurse educators and dietitians and cover many topics related to living with diabetes, including:
- Goal setting
- Medications
- Monitoring, prevention, detection and treatment of acute and chronic complications
- Nutrition management
- Physical activity
- Problem solving
- Psychosocial adjustment
- Understanding the diabetes disease process
Loyola’s nurse educators and dietitians, working closely with each patient's doctor, develop individualized care plans.
Individual discussions include the areas covered in group classes, as well as preconception care, management of pre-existing diabetes in pregnancy, gestational diabetes and intensive insulin therapy, including insulin pumps and continuous glucose monitoring with sensors.
