Compassionate and Comprehensive Home Care
For patients recently discharged from Loyola University Medical Center who may be experiencing illness or injury and will be recovering at home, Loyola Medicine Home Care provides compassionate and comprehensive care.
Loyola’s skilled doctors, nurses, therapists and home health aides provide care that focuses not only on medical issues, but also emotional and spiritual needs. Care is provided in the patient's home, with inpatient care for symptom control and pain management provided at Loyola University Medical Center.
The home care program provides comprehensive care for patients at home, which may include:
- Assistance with community resources
- Cardiac treatment and education
- Diabetes care
- Diet management
- Disease management
- Physical therapy
- Occupational therapy
- Medical social services
- Medication and health teaching
- Rehabilitation services
- Skilled nursing
- Speech and language pathology
- Tracheostomy care
- Wound care
Loyola Medicine Home Care is a Medicare-certified, Illinois Department of Public Health-licensed home care provider. The program is accredited by the Community Health Accreditation Partner (CHAP).

Home Infusion Pharmacy Program
For home care patients, Loyola’s home infusion pharmacy program provides access to many medical therapies in the comfort of your own home, including:
- Antibiotic, antiviral and antifungal medications
- Chemotherapy treatment
- Fluids for hydration
- Immunosuppressive medications
- Immunotherapy
- Medication for blood disorders
- Medications for cancer patients
- Medications for cardiovascular conditions
- Pain management therapy
- Steroids for various conditions
- Total parenteral nutrition (TPN)
Bone Marrow Transplant Home Care
A bone marrow transplant (BMT) home care program provides skilled and high-tech nursing services to pre- and post-transplant BMT patients. This service allows patients to be cared for in their homes, reduces length of a hospital stay and increases patient comfort. Care is provided by a team of highly skilled, BMT cross-trained nurses.
Home Medical Equipment (HME)
The HME program provides equipment necessary to support care at home, including:
- Assisted daily living (ADL) aides
- Assistive devices
- CPAP machines (continuous positive airway pressure)
- Enteral nutrition
- Hospital beds
- Oxygen
- Wheelchairs
